FIRST VISIT:
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Please allow one hour for this initial visit.
Please assist us by providing the following information at the time of your consultation:
- Patient Information Form (you can download and print from our "Forms" page)
- A list of medications you are presently taking
- If you have dental insurance, please bring your card for us to copy and verify. We must have the name, social security number and birthdate of the Insured at this visit.
IMPORTANT: All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.)
X-RAYS:
A series of x-rays may be taken for us to determine your treatment plan.
